Forms
A form in GoHighLevel is a standard lead capture tool. You drag and drop fields: first name, last name, email, phone, dropdown menus, checkboxes, and custom fields you have created. You style it to match your brand.
Forms submit to GoHighLevel and create or update a contact record. The submission triggers any workflow that uses "form submitted" as its entry event. You can also redirect the visitor to a thank-you page after they submit.
Surveys
Surveys present one question at a time, like a Typeform-style experience. They are better for longer intake forms or qualification questionnaires where you want the person to stay engaged question by question rather than seeing a full form at once.
Surveys support branching logic: if the answer to question 2 is "yes", show question 3a. If "no", show question 3b. This lets you build smart qualification funnels that only ask relevant questions.
Using submissions in workflows
Every form and survey submission is tied to a contact. The data from each field lands in the corresponding contact field in the CRM. Workflows use this data to personalize follow-up, route leads to the right team member, or make decisions in automation logic.
